Hubungan Antara Kadar Serum Laktat dengan Karnofsky Perfomance Status, Derajat Keganasan dan Ukuran Tumor pada Pasien Tumor Primer Intrakranial di RS Adam Malik Medan

Laktat berperan sebagai produk metabolik utama dari glikolisis anaerob pada sel tumor otak yang mengalami efek Warburg. Peningkatan kadar laktat mencerminkan aktivitas metabolik, hipoksia jaringan, dan agresivitas tumor, serta berpotensi sebagai biomarker prognosis keganasan tumor primer intrakranial. Mengetahui hubungan antara kadar serum laktat dengan dengan skala Karnofsky Perfomance Status (KPS), derajat keganasan dan ukuran tumor pada pasien tumor primer intrakranial. Penelitian ini menggunakan desain cross sectional pada 32 pasien tumor primer intrakranial di RS Adam Malik Medan yang memenuhi kriteria inklusi dan eksklusi dilakukan pengambilan data identitas, penilaian status fungsional pasien dengan Karnofsky Perfomance Status (KPS), derajat keganasan tumor berdasarkan hasil histopatologi dan ukuran tumor berdasarkan hasil pencitraan radiologi. Dari 32 subjek penelitian, didapatkan mayoritas pasien dengan derajat keganasan tumor rendah/low-grade (71,9%), nilai median ukuran tumor 40,81 cm3 (2,41 - 353,56 cm3), KPS 70 (40-90), kadar laktat serum 2,4 mmol/L (1,41 - 6,66 mmol/L). Berdasarkan uji korelasi Spearman, terdapat hubungan yang signifikan antara kadar laktat terhadap derajat keganasan tumor dengan kekuatan korelasi kuat (r = 0,772, p < 0,001). Namun, tidak dijumpainya hubungan yang bermakna antara kadar laktat dengan skala KPS dan ukuran tumor pada pasien tumor primer intrakranial dengan nilai p = 0,195 dan p = 0,903 berurutan. Kadar laktat serum berkorelasi positif dengan derajat keganasan tumor primer intrakranial.
